Step 2: On the homepage click on the “Schemes” option. The list will appear on your screen.

Step 3: Now, click the “Click For more” link beside the “Dr. Ambedkar Medical Aid Scheme – Revised in 2018” section.

Step 4: Next, click the “Click For More / Print” option located at the bottom of the page.

Step 5: Download the Application form for Medical Aid under Dr. Ambedkar Medical Aid Scheme (for SC and ST only).

Step 6: Fill out the prescribed application form or use plain paper according to the details in the proforma. The application format is attached as Annexure-I.

Step 7: Submit the following self-certified copies with the application form such as Caste Certificate, Income Certificate, Ration Card or Aadhaar Card, Estimated cost of surgery, certified by the Medical Superintendent (see Annexure-II).
